The specialised services of Community Mentors provided by Liberty Disability Services ensures the NDIS participant can access the necessary supports to reach their goals, access the community, build capacity and sustain independence. As a Community Mentor you will provide support and assistance using friendly and transparent forms of communication to ensure qualitative services to participants and team members. You will provide in and out of home supports in a range of supports and activities, to allow the best possible outcomes can be achieved for people with a disability including children, young people and adults. Recognising each individual’s disabilities, abilities and personal circumstances, your overall supports will actively seek involvement in the following:
● Encourage community connection and independent living skills
● Provide practical support’s and encouragement to access the community, social groups and 1 to 1 opportunities outside of the participants home
● Provide practical assistance towards independent living skills within the home As a representative of Liberty Disability Services your primary focus is:
● Encourage each participant to build capacity and independence, allowing choice and control
● Allow freedom of expression for each participant that is free from judgement
● Motivate participants to seize every opportunity to pursue their goals
Prerequisites of role:
1. Nationally recognised police check
2. Current Victorian Driver’s License and a well maintained vehicle that meets current safety standards
3. Comprehensive Car Insurance
4. Current Working with children check
5. Mobile Phone in good working condition with internet and camera capabilities
6. Current First Aid & CPR Certificates

Expressions of Interest
A Recovery Coach support is designed to be able to maintain engagement through periods of
increased support needs due to the episodic nature of mental illness. Recovery coaches work
collaboratively with participants, families, carers and other services to identify, plan, design and
coordinate NDIS supports.
Recovery coaches will provide support to people with psychosocial disability to live a full and
contributing life. NDIS participants will be assisted to take more control of their lives and to better
manage complex challenges of day to day living.
● Through strong and respectful relationships and skilled coaching, people will be
supported to build capacity, including strengths and resilience.
● Recovery coaches will work collaboratively with participants, their families, carers and
supports to design, plan, implement and adjust a recovery plan.
● They will collaborate with the broader service system and will assist with the coordination
of NDIS and other supports.
Recovery coaches are different from support coordinators in that they bring specialist
knowledge and skills in psychosocial recovery, mental health and service navigation within the
mental health system.
Roles and responsibilities of recovery coaches include:
● Developing a recovery-enabling relationship
● Supporting participants’ engagement with the NDIS
● Coaching to increase recovery skills and personal capacity, including motivation, strengths,
resilience and decision-making
● Supporting participants with their recovery planning
Collaborating with the broader system of supports.
Prerequisites of the role:
Tertiary qualifications in peer work or mental health (minimum of Certificate IV in Mental Health Peer Work or
Certificate IV in Mental Health) or equivalent training; and/or a minimum two years of experience in mental health-related work.

Recognising each individuals disabilities, abilities and personal circumstances, your overall supports will actively seek involvement in the following:
● Communicate, coordinate and maintain relationships with support providers necessary for the individual participant’s journey toward reaching goals.

● Manage and utilize the funding provided in the participants NDIS plan according the NDIS Operational Guidelines in conjunction with the participants NDIS goals As a representative of Liberty Disability Services your primary focus is:
