What is Support Coordination?
Support Coordination is a service offered by a disability provider that assists participants to use their NDIS plan and funding in the best way possible.
If your NDIS plan includes Support Coordination, you will be paired with a Support Coordinator. The role of this Support Coordinator is to help you coordinate your supports (such as therapy, personal care, or community access) required in your everyday life and make sure they are implemented in a way that suits your individual lifestyle. The Support Coordinators number one goal should be to enhance your life with supports that empower you!
Working with a Support Coordinator can assist you to:
- Understand and use your NDIS plan to accomplish your unique goals.
- Connect you with a range of service providers, community, mainstream and other government services.
- Build your confidence and skills to use and coordinate your required supports in a manner that suits you best.
- Coordinate your plans funding allocations.
- Pick and choose who gives you services
- Live more independently
- Feel more included in your community
- Make sure your choices and wishes are respected.
What is Pathways Support Coordination?
Liberty’s Support Coordination Pathways – is a service offered by Liberty’s Pathways Support Coordinators experienced in supporting people with a disability who are in need of the following services::
- Hospital Discharge
- Homelessness and Outreach.
If your NDIS plan includes Support Coordination, and you require a specialised skillset from our support coordinator team, we can pair you with a suitable Pathways Support Coordinator. The role of a general Support Coordinator is to help you coordinate your general supports (such as therapy, personal care, or community access) required in your everyday life, a Pathways support coordinator with will guide you through the additional support you need to make sure they are implemented in a way that suits your individual lifestyle. The Pathways Support Coordinators number one goal should be to enhance your life with supports that empower you!
Pathways Support Coordination- Hospital Discharge
Liberty Pathways Support Coordination is a specialised team focused on organising and overseeing a secure and seamless transition from hospital or other residential care settings to your home or a new home. Our pathways care teams are headed by an experienced leader in Support Coordination, alongside skilled disability support specialists and therapists, and are proficient in intricate disability and medical assistance.
Our team collaborates with you, your family, the hospital, your current residential care provider, and your multidisciplinary team to:
- Be flexible to meet in the hospital
- Be involved in large care team meetings
- Coordinate the services required to leave the hospital and return home in a supported and safe environment
- Develop a home transition plan and the assistance to implement it
- Continuous support as you transition into a new lifestyle
Helping you achieve your goals and live life on your terms, will be a team enriched with different skills and perspectives. We work with your team to make sure your support and care plan is right for you, and that your support team knows how to help you in the best way. Meetings with your team are an important part of how we help you transition and manage your care.
Our procedure encompasses all crucial stages essential for a smooth transition home. Starting with facilitating early supports between you and the support team, proceeding through connections, establishment, and training on the necessary equipment for your well-being at home. The process is structured to ensure readiness for you, your team, and your family upon transitioning to your new way of life
Pathways Support Coordination- Homelessness & Outreach
Liberty’s Pathways SC program is dedicated to assisting individuals who are currently experiencing or are at risk of homelessness. This initiative offers comprehensive support to guide individuals through the challenging transition into stable and sustainable housing. By accessing NDIS funding or other funding supports and providing a range of resources, counseling, financial guidance, vocational training, and access to community services, the program aims to empower you to overcome barriers and establish a secure foundation for your future. Through personalised support and advocacy, Liberty’s Pathways SC program strives to ensure that every individual receives the assistance needed to achieve housing stability and improve overall well-being.
A Pathways support coordinator can:
- Be flexibility to meet at Rosshouse or various locations
- Involved in large care team meetings
- Have an understanding of additional barriers a person may face
- Find supports to assist you through these barriers
- Assist you to find providers for assessments and diagnosis
- Connect with new providers of your choice
- Help you search for your medical history
- Connect you with housing supports
Who needs it?
Pathways Support Coordination is recommended to anyone who seeks help in the coordination of their NDIS plan and the additional supports they use/require to move through the barriers and sustain a positive lifestyle.
This may include participants who are in hospital or homeless and:
- Are new the NDIS or have just received their first plan
- Require assistance in engaging with mainstream disability services and providers.
- Are unfamiliar with how funding allocations work within an NDIS plan.
- Require assistance from someone to find and connect with the RIGHT service provider… A provider who truly understands and supports you as a unique person.
What do I need in my plan?
To access Pathways Support Coordination with an NDIS plan, you will need funding under the Capacity Building Category- Support Coordination.
This funding is based on what is ‘reasonable and necessary’ (NDIS) to pursue your individual goals, in addition to the support provided by family, friends, and other community and government services.
The funding will be a fixed amount for a Support Coordinator to work with you regularly- helping you confidently utilize your NDIS plan.

If you have an approved NDIS Plan and are seeking Support Coordination, contact your Local Area Coordinator (LAC) or NDIS Planner to find a Support Coordinator that suits you best.

What does it cost?
